Sponsorship Information Release Form

The University of Kansas has a pending agreement from a third-party organization or sponsor that will pay all or part of specific tuition and/or fees. By completing this form I acknowledge that the University may:

Bill my sponsor(s) with KU's standard Sponsor billing of the eligible tuition and fees.
Release information to the sponsor(s), such as KU student account information, enrollment, transcripts, certifications, diplomas, general financial aid award, and filing information in accordance with University policy.

This form must be completed by the sponsored student.

A temporary credit is applied to my student account while KU bills my sponsor for the authorized charges. This credit does not represent actual payment from my sponsor and can be removed from my account if my sponsor fails to pay.

I understand I am responsible for any remaining charges in the event that my sponsorship changes or fails to pay for any reason.

I understand that unpaid charges may cause:
Late fees and/or collection costs
Inability to enroll in future semesters until all charges on my KU account paid in full.

I understand that sponsorships for future terms are not allowed until current sponsorships are paid in full.

I authorize The University of Kansas to release student account information and/or student academic record necessary to service my account to my sponsor. I understand that this electronic signature has the same legal status as a manual signature.
