Religious Accommodation Form

Students should submit the Religious Accommodations Form to inform instructors of the day(s) you intend to be absent due to a religious observance. You should submit the form for each course and each holiday within the first 20 days of class.

Fill out the form, and include the name and dates of the observance for which you are requesting a class exemption. An email will be sent to notify your instructor. From there, your instructor should follow up with you regarding your religious observance.
