This survey is only to request writing consultation appointments for Kansas Law Enforcement Training Center students, Haskell students or recent KU alumni who are not able to access the KU single log-in required for our appointment system. If you are a current KU student, do not use this form to sign up for an appointment. Instead, use Jayhawk GPS or the Navigate app to make your appointment.
