Letters of Recommendation Request Form

Please complete the HERS summer internship program letters of recommendation request form. This form requires the contact information for an academic reference and a character reference. Once this form is submitted, an email with instructions will be sent to each of your references.

Both the online application and the letters of recommendation form must be completed for applicants to be considered. HERS program online applications and letters of recommendation forms must be submitted by 11:59pm (CST) on March 15, 2019.

You are not required to submit an online application before completing this form. We highly recommended that applicants complete this form as soon as possible to give references adequate time for thoughtful responses.

NOTE: your responses will not be saved. Please complete only when you have all necessary information for both references.
